Child Health Clinics: held every 3rd Thursday of the month. Infants six weeks to children through age 11 are eligible. Older children are seen for camp physicals. Our office DOES NOT give sports physicals. Payment is by sliding scale according to your income. No one will be denied services due to inability to pay.
Immunization Clinics: held the first Wednesday, third Wednesday (afternoon) & fourth Monday of each month. All clinics are by appointment. Age appropriate vaccines are administered beginning at age two months of age. TD (Tetanus) is available for anyone over the age of seven. There is a $5.00 fee for children and a $15.00 fee for adults charged for each immunization. No one is denied services due to inability to pay.
Presumptive Eligibility Site for CHP+ and Medicaid: CHP+ and Medicaid application assistance
Vital Records: local vital records office for registering and issuing birth and death certificates.
Healthy Communities: an outreach and case management service which connects Medicaid enrolled families to resources and services in the community. Also assists families to get treatment for medical, dental, vision, mental health and developmental problems.
Tobacco: provides information regarding resources and referral for adults interested in cessation services. Tobacco Quit-Kits available free of charge for inhaled tobacco and chew tobacco. Pregnancy specific Quit-Kits also available. The Tobacco Coordinator is available to answer questions regarding tobacco use and cessation.
Well Adult Clinic: held monthly at the Garden Park Villa, the Royal Gorge Manor and Coaldale. Individuals must be over 60 to receive this service. There is no charge for this service, but a donation is requested.
Health Care Program for Children with Special Needs: HCP is a federal and state funded medical assistance program for children up to 21 years old that have a physical disability, which interferes with their normal growth and development. Our office provides case management for this program.
Choose To Be! Tobacco Free!
Choose to Be Tobacco Free is a program for pregnant women who smoke and want to quit. A woman who quits by the time she delivers is eligible for a $25 diaper voucher for each month she remains tobacco free (after delivery). She can earn up to $300 worth of diaper vouchers. A breath test is used to determine tobacco free status.
